We strive to provide you the best, personalized care available. To make this possible we adhere to a set of very important guidelines. Please read them carefully.
The day before your first appointment we will call you to confirm. We need your verbal confirmation by 4:00 pm. If we do not hear from you, your appointment will be cancelled. We will leave a message so if you happen to miss our call, please give us a call back.
You will automatically receive an email confirmation the day before your appointment if your email is provided during the intake process. You may opt out of reminders altogether. Just let us know!
You will be given a check-in time of no less than 10 minutes prior to your appointment. We keep a strict schedule in order to allow for timely treatment for all of our patients. If you are not in the lobby ready to be checked in at the time of your appointment, your appointment will be rescheduled and a no-show fee may be charged.
If you wish to change or cancel an appointment, we require a minimum 24-hour advance notice. Anything less will result in a $50 fee charged to your account. It costs us money to make appointments available to you. Whether you attend or not we still accrue expenses (for staff wages, rent, etc).
We do not charge you the actual cost for that appointment but rather mere $50 fee. We do NOT make money with this charge; it's only to act as a deterrent from making last minute changes.
Advance notice allows someone else (who needs it) time to reserve it in place of you. Please be courteous and responsible. Thank you.
We accept cash, check, or Venmo® with no fee. Credit and debit cards may also be used; keep in mind there is a flat rate convenience fee of $3.95 that will be assessed to the total due. We recommend bringing cash or using Venmo® to avoid these charges.
If you fail to show for an appointment without notice all future appointments will be removed from the schedule and a $50 fee assessed to your account. You may reschedule appointments again on a "first come, first serve basis." No-shows on your initial evaluation will result in your case being closed, your referral disregarded, and your case referred to another clinic. Your physician will be notified.
Rationale: Respect is one of our Core Values. We understand that life happens that can prevent you from making it to your appointment on time; however, we expect OPEN COMMUNICATION and the courtesy of a phone call so that we may work with you. We may be able to work some scheduling magic in order to get you in. Do us the courtesy of keeping us posted on what's going on since your therapist will be waiting for you.
We realize emergencies may arise and therefore allow you to carry your cell phone during your session, however, please be courteous and set to silent mode or turn off. No cell phone use is allowed in the gym area due to patient privacy and confidentiality.
Our facility does not offer child care services. You may not bring children who require supervision with you to your appointment. If your child does not require supervision and is capable of waiting for you quietly then you may bring them. If any disturbance is caused to other patients or staff members, you may be asked to terminate your session early and attend to your child.
If you are experiencing financial difficulties and are unable to afford the cost of our services, we have a "Financial Hardship Application" which may be filled out. If you qualify for financial assistance according to the Federal guidelines, we may legally assist you by waiving or discounting your (patient responsibility) portions of the bill.
Ask the front desk for assistance. Generally, if you qualify for Medicaid, you will qualify for Financial Hardship as the guidelines are the same.
