We accept all insurances!

Paying out of pocket. You can view our cash packages under the Services section. 

1. Have your doctor fax us a referral

2. Once we receive your referral we will give you a call 

3. You will need to fill out paperwork (you can pick it up or we can email it to you)

4. After we receive your forms we can set up your first appointment 

Please wear comfortable clothing (make sure we are able to get to the area being treated). Your physical therapist will do tests and measures and will create a home exercise plan for you, so expect homework. At the end of your appointment the doctor will let you know how many times per week you need to come in and the front desk will schedule your appointments according to the plan. 

Sure, cupping is $25 for 30 min and it would have to be scheduled as a separate appointment. This service alone is not covered by insurance. Cupping maybe included if the clinician deems it necessary. 

Yes, if you pay out of pocket. If you're using insurance, manual therapy is part of the PT program.
